Serious Incident Prevention by Tom Burns

Drawing from his long service with a large chemical company, consultant Burns presents a new approach to preventing serious incidents through safety management processes, in contrast to the old approach of mere compliance with regulations. Among the elements he describes are management commitment and leadership, employee involvement, identifying critical work, establishing standards, measurement and feedback, and improving and updating the process. He concludes with a hypothetical case study. The first edition appeared in 1999.
